INCIDENT SITE INVESTIGATION

The ‘Incident Site Investigation’ Service provides photographs, measurements, maps, sketch plan (if relevant) with Section 58 Witness details and evidence to the accident site or defect (‘Witness to Defect’ form).  This would include Slip and Trip defects with a completed and signed brief questionnaire by a witness to the defect.

• The client should be in attendance at the accident site wherever possible
• Several photographs of the defect and the surrounding area. One photograph should have the client pointing at the specific defect
• A map of the area and at least one witness to the defect form. If the defect is not actionable, the agent should contact LSS before doing any work
• Detailed sketches with measurements should be provided, if appropriate and relevant

Photographs

• A set of photographs of any site in a photograph template format.

Incident Site Investigation & Collection

• A map of the area and at least one witness to the defect form. If the defect is not actionable, the agent should contact AIS before doing any work.

After the site visit

• Visit the client and collect personal and address ID, ID sheet and ‘True Likeness’ form
• Thorough investigation of LEI with LEI checklist and clear copies of insurance documents
• Explaining the funding process
• Complete the CFA checklist
• Explain, get signed and collect any relevant documentation sent by the solicitor
• A photograph of scarring, if relevant and appropriate
