Due to ‘fixed fees’ our RTC Questionnaire offers all the information supplied in the Full Statement Service but with a detailed hand-written questionnaire, which will be signed by the claimant during our visit, rather than a fully typed statement.

 

Road Traffic Accident Questionnaire - Includes RTA 1 (Claimant)  & RTA 2 (Defendant)

 

  • Production of a legible hand written questionnaire which is signed by the client
  • Personal and address ID, ID sheet and ‘True Likeness’ form
  • Investigation of LEI with LEI checklist and clear copies of insurance documents
  • Explain the funding process
  • Complete the CFA checklist
  • Explain, get signed and collect any relevant documentation sent by the solicitor
  • Provide photographs of the accident site (if required)
  • A photograph of scarring if relevant and appropriate
  • Maps, a sketch of the accident site and witness details.

 

Slip & Trip Questionnaire


  • Production of a legible hand written questionnaire which is signed by the client
  • Personal and address ID for the client, ID sheet and ‘Tue Likeness’ form
  • Thorough investigation of LEI with LEI checklist and clear copies of insurance documents
  • Explain the funding process
  • Complete the CFA checklist
  • Explain, get signed and collect any relevant documentation sent by the solicitor.
  • Sketches (if relevant) and a photograph of scarring if relevant and appropriate.

 

Employer Liability Questionnaire

  • The client should be in attendance at the accident site wherever possible
  • Several photographs of the defect and the surrounding area. One photograph should have the client pointing at the specific defect If the defect is not actionable, the agent should contact AIS before doing any work
  • A map of the area and at least one witness to the defect form
  • A photograph of scarring, if relevant and appropriate
