FULL STATEMENT SERVICE

A visit is made at a convenient place for the claimant, explaining the CFA process completion of a comprehensive LEI investigation.  In addition, to help the Client recover costs, the instruction agent will include a ‘Time Sheet’ showing how long each aspect of work has taken with any added costs incurred.  The time sheet is valuable to an injury law firm when recovering costs.

Instruction agents are trained in fraud and will rigorously vet and quality check statements.  Only after the statement has passed the checks will the instruction agent email the final version of the statement to the Client.

Slip & Trip Full Statement

• Production of soft copy and a printed statement signed by the client,
• Personal and address ID, ID sheet & ‘True Likeness’ form
• Thorough investigation of LEI with clear copies of insurance documents & LEI checklist
• Explaining the funding process and completing the CFA checklist.
• Explain, get signed and collect any relevant documentation sent by the solicitor.
• The client should be in attendance at the accident site wherever possible.
• Several photographs of the defect and the surrounding area. One photograph should have the client pointing at the specific defect. If the defect is not actionable, the agent should contact AIS before doing any work.
• A map of the area and at least one Witness to the Defect form if the defect is in the public highway.
• A photograph of scarring if relevant and appropriate
• Sketches (if relevant)
• Any appropriate witness details should be included.

Non Slip & Trip Full Statement – this includes

• Production of soft copy statement
• A printed statement signed by the client
• Personal and address ID  & ID sheet & ‘True Likeness’ form

• Completion of the CFA checklist

Witness statement

• Production of a soft copy and a printed statement signed by the witness
• Witness ID but, if the witness refuses the agent does not press the issue

Industrial Disease Statement

• Production of a signed statement
• Personal and address ID for the client
• Investigation of LEI as far as is possible (accident dates usually quite old)
• Explaining the funding process and completing the CFA checklist
• Explain, get signed and collect any relevant documentation sent by the solicitor.
• Provide photographs of the injuries (if appropriate) and any relevant witness details.

Quantum Only Statement

Production of soft copy and a printed statement signed by the client along with personal ID for the client

No requirement for LEI Checks or CFA explanation
